The origin of the current excitement in utilizing extracorporeal life support in critical care medicine can be traced to a newborn infant in California over 35 years ago. Since that day when Dr Bartlett introduced this life saving technique in the Neonatal ICU and reversed the expected outcomes in infants with pulmonary hypertension from death to survival, the practice of ECMO has evolved. New medical devices and pharmacological agents have reduced the need for ECLS in the Neonatal ICU, but the application of extracorporeal support has expanded to other areas, especially into the cardiac ICU. The acceptance of ECMO in pediatric critical care medicine has been much slower, but it's use continues to increase following a similar path as the adult ECMO numbers. In this talk, a historical perspective of ECMO will reviewed to allow a perspective of the ECLS that we practice today. A review of the technology advances and the data emerging from recent publications will be discussed to offer a glimpse into the future of ECLS support in Neonatology and Pediatric Critical Care.
