Surgical Treatment of Dermatofibrosarcoma Protuberans

Dermatofibrosarcoma protuberans (DFSP) is a rare, slow growing, low grade sarcoma originating in the dermis and subcutaneous tissue with a nodular picture. Decision of safe margin for excision is important for decreasing the recurrence rate. We have 46 patients with the pathological diagnosis of DFSP during the past 15 years (1979-1993). The treatment of choice was wide excision with a safe margin of at least 3cm. Thirty three patients were followed for more than 3 years. The recurrent rate was 15.15% (5/33). In our series there were no recurrence of whom were free from tumor more than 3 years after the first operation. So regular follow up is particularly important in the first 3 years. We conclude from this series, and also from review of the literature, that wide local excision offers an excellent probability of cure, and the radiation therapy is only a reasonable alternation in patients who have unresectable lesions.
