Analysis of Prostatic Calculi at Taipei Veterans General Hospital

OBJECTIVE: To analyze the composition of prostatic calculi, and their association with serum prostate-specific antigen (PSA) level, weight of the resected prostatic adenoma, urine white blood cell (WBC) count, and urinary tract calculus formation. MATERIALS AND METHODS: We analyzed the compositions of prostatic calculi retrieved from 31 consecutive patients after transurethral resection of the prostate using infrared spectroscopy. We used non-parametric and Fischer’s exact test to analyze their association with serum PSA level, resected weight of the prostatic adenoma, and urine WBC count. We used intravenous urography (IVU) reports. Urinary tract sonography reports, and cystourethroscopic findings to analyze their association with urinary tract calculus formation. RESULTS: Endogenous prostatic calculi comprised 32.2% while exogenous prostatic calculi comprised 16.1% of all calculi analyzed. Half of the prostatic calculi (51.6%) were a combination of calcium phosphate and calcium oxalate. A proportion of patients with prostatic calculi (9.6%-19.3%) had previous urinary tract calculi. Increasing the weight of the resected prostatic adenomas was associated with more than 1 composition of prostatic calculi. Serum PSA level and urine WBC counts were not associated with prostatic calculus formation or calculus-induced prostatitis. CONCLUSIONS: Nearly 1/3 of prostatic calculi (32.2%) at Taipei Veterans General Hospital were endogenous in origin. About 1/6 of prostatic calculi (16.1%) were exogenous in origin. Half of these calculi (51.6%) had a mixed composition. A proportion of patients with prostatic calculi (9.6%-19.3%) had previous urinary tract calculi. Increasing the weight of the resected prostatic adenomas was associated with more than 1 compostatic of prostatic calculi. Serum PSA and urine WBC counts were not associated with prostatic calculus formation or prostatitis.
