Thyrotoxic Periodic Paralysis: A Case Report

Thyrotoxic periodic paralysis (TPP) is manifested as recurrent episodes of muscle weakness, involving mainly the extremities ,lasting from hours to days. Hypokalemia and thyrotoxicosis are usually present during attacks. Although not a very common presentation of hyperthyroidism, TPP is the leading cause of hypokalemic periodic paralysis (HPP) in Asian populations. For patients presenting with hypokalemic paralysis in the emergency room (ER) , TPP should be included in the differential diagnosis and the thyroid function tests should be checked. Treatment of the underlying hyperthyroidism cures the TPP ;and potassium and propranolol can be used with caution to hasten recovery in the ER. We report on a 41-year-old Chinese man with TPP successfully managed by potassium administration ,and throidectomy.
