Normal Splenic Size in Adults: Ultrasonographic 2-D Index

This study was conducted to establish a simple guideline for assessing normal splenic size in adults of different ages by using a real-time ultrasonographic 2-dimensional index (US2DI). A study of 495 consecutive adults, including 324 males and 171 females, was done. Their ages ranged from 20 to 90 years, and they were divided into 7 groups from the 3rd to the 9th decade. No patients had a history of disease related to the liver or spleen. The maximal length (L) of the spleen was obtained by oblique scan through the left posterior axillary line along the left lower intercostal spaces. The width (W) was measured perpendicularly from the hilum to the outer splenic capsule, and the US2DI was defined as the product of L and W. The following guidelines for US2DI were based on the product of the length and the width: 20-29 years: male, 44.21±9.44 (n=6) female, 41.09±9.57 (n=16) 30-39 years: male, 45.01±11.78 (n=58) female, 43.27±11.69 (n=22) 40-49 years: male, 46.24±9.36 (n=58) female, 40.54±13.29 (n=32) 50-59 years: male, 42.14±8.60 (n=58) female, 44.28±10.11 (n=42) 60-69 years: male, 43.30±11.71 (n=77) female, 43.72±10.71 (n=40) 70-79 years: male, 42.18±10.78 (n=62) female, 40.26±9.55 (n=17) 80-89 years: male, 55.28±15.15 (n=5) female, 50 (n=2) Overall (20-89 years): 43.10±11.60 (n=495) This US2DI may be used to assess splenic size in adults.
